It's not supposed to: There is a constant interplay between clotting and anti-clotting factors in the blood stream. Eventually, clotting will occur, but not during normal or even "extended" erections, whatever those are. That's why the tv commercials tell you to seek medical attention for an erection lasting more than 4 hours.
