See his doctor: Can't even guess what the problem might be. If he has been sexually active, STDs obviouly are a concern and might be what he fears. No matter the cause, he obviously needs to see his pediatrician. How fortunate your son is OK speaking with you about it! Be sensitive and caring, and assure him he may see the doctor without telling you more, and without you being present.

Penis Anxiety: As another doc said, please take your son to the doctor. It could be one of any number of things, many minor, but you need to know. It may even be that you son wants to make sure that he he “normal” in that area. You are doing something right in that he told you. Peace and good health.
