Exam: Exam would usually be able to determine if hernia present and differentiate it from the other entities you mentioned. If any question, ct scan can also help confirm diagnosis. I agree with comment that if you are uncomfortable proceeding, don’t hesitate to get second opinion.

Rare things are rare: Any surgeon worth their certification can tell the difference between a common hernia & a rare spermatic cord defect. Robotic surgery is the state of the art repair approach in most cases. Most can have surgery, be home that day & drive the next with this method. If you don't like or trust the surgeon get another opinion.
