Physical exam: About 1/2 or more males will have transient gynecomastia as they transition through puberty. This can last 6mo-1yr in most with a few lasting longer. Sponginess of the glandular tissue under the nipple is one way to separate actual glandular tissue from fat, but telling the difference takes training.
