Risk for....: The idea of looking at possible risk factors for something involves planning for prevention. For example, risk of falling, once the nurse determines it, can start a series of measures such as using walking aids to help prevent falls, or having someone assist with transitions from bed to chair, or starting physical exercises that help with balance.
