Rarely, but yes: Fallopian tubal sterilizations have a failure rate of 1 in 500 to 1 in 10,000 depending upon how they were interrupted. When failure occurs it is usually due to re-canalization whereby the tube puts itself back together again by going around the obstruction or the "clip" falls off or was misplaced initially. Failure often result in ectopic pregnancies.
