There are any of: a number of conditions that could be causing your symptoms but the most likely ones are due to your weight. You probably have pulmonary hypertension as this will cause chest pain (tightness) cough and cardiac irritability leading to skipped beats. You may have developed right heart failure as well. You need to get professional help to lose weight or your life expectancy will be severely shortened.
