Do i need to discuss an advance care plan with anyone?

Your family and GP: It is important that your family and those closest to you know your wishes if you are very ill. We all think differently about life; even a close relative or friend does not necessarily know or understand your wishes for your future medical care. Talk to your doctor, who can explain medical terms and your own health progress to you or a healthcare worker involved in your regular health checks.
