Not fungal?: It may not be fungal but a bacterial infection. Your doctor may want to try an antibiotic or do some testing.

Irritant dermatitis?: May be the cause and come and go and not respond to antifungals or antibacterials as it is an irritant phenomenon. Needs a good examination of the rash by a doctor. They can then arrange swabs and fungal scrapings if required +/- treatment +/- dermatology opinion if needed to sort you out. All the best.
