Sees doctor: If you have tried barrier creams such as Sudocream already and the rash is still persisting or worsening it is a wise idea to see your GP. Sometimes nappy rash can get infected and may need antibiotics or antifungal creams on top of normal barrier creams.

Needs a visit: The wrong treatments for any rash would seldom help. A review of facts like age, location, aggravating factors, appearance are all needed to sort out a rash. You haven't provided enough of that information for us to do so here but the kids doc can in an office visit.
