It could be: time to close your computer and proceed to the ER or UC clinic. Otherwise, I would suggest a consultation with both an ENT and a neurologist.

History: Have you been evaluated by a doctor for your vertigo? I assume that you have given you are using Epley Maneuver to try to work through the BPPV. Need more history to understand whether the twitching started after or upon onset of vertigo. Please see your treating physician promptly for further evaluation.
