Night terrors: This is most likely related to night terrors, a disorder of stage 4 sleep, like sleep walking. The age is right and the waking with sudden screaming. However, the soaked pillow and wrinkled hand is a bit odd. Being disoriented is an odd combination as well, though it's tough to say if it's due to waking or another reason. I'd consult his doc asap and run this by them for real medical advice.
