Temperature: It would be nice to know the source of the temperature and the age of the baby but you are right it is high. Have him seen particularly for less than 3 months and if older particularly if he does respond to the antipyretic and has other symptoms. All the best. Do not over bundle.

Yes-take your baby to an ER or Urgent Care center as quickly as possible!--A temperature of 39.6°C=103.28°F & the normal range for a baby is 36.5 to 37.4 degrees Celsius or 97.5 to 99.3 degrees Fahrenheit. Your baby may be suffering from an infection or other illness which should be treated as soon as possible. I hope for the best for the health of your baby!
