Liver pain: Two possibilities. (1) pressure upon the liver capsule (outer lining of the liver), which is sensitive to pain. (2) blockage of ducts within the liver with resultant back pressure.

They don't: Small liver cysts that do not stretch the capsule of the liver should not cause pain. Large or multiple cysts can stretch the capsule, which can lead to pain.
