Rectovaginal fistula: Presumably you are under care for your rectovaginal fistula. Follow your doctor's advice about treatment, including pain control. If for some reason you are not in care for it, see a doctor ASAP.

Bleeding : Not sure whether you mean you are bleeding from an anal fissure or from your vagina. Suggest best to see a doctor as sounds like you may need an examination to sort out what is going on
