Drug interaction : Topamax more than 200 mg / day would affect hormonal contraception and decrease its effectiveness, use an additional contraceptive method / decrease topamax dosage, through your prescriber. Breakthrough bleeding could be a side effect of the drug-drug interaction, see your doctor(s) for evaluation, best wishes
