No: A biopsy's appropriate whenever the diagnosis is in question. Different dermatologists will have different levels of confidence without biopsy, and biopsy practices will differ. No dermatologist will biopsy dandruff or acne. Wide excision is best for anything suspected of being a melanoma. Best wishes.
