Effective: Plan B consists of hormones, which, given early after unprotected intercourse, "mess" with the mechanisms that are needed for successful conception and/or implantation of a fertilized egg. Under ideal circumstances, conception rate is only about 25 % to begin with. The only way to be sure that Plan B was effective is to do a urine pregnancy test the earliest a week after the unprotected IC.

Yes: If you aren't ovulating then you can't get pregnant from unprotected sex. If you ovulated before condom broke, then Plan B can't help. Sperm lives for up to 3 days, so it's always a good idea if you don't want to be pregnant to take Plan B asap. If taken right away it prevents pregnancy up to 85% of time.

Very little chance: but very little chance as you took plan B . You did right thing if you don't want to. . If you want to make sure get a pregnancy test done , if pregnant will be positive with in 10 days. Good Luck
