Why do some doctors prescribe a hard collar after acdf but others do not? what is the current philosophy?

Belt and suspenders: The collar is to remind the patient to limit their neck motion and avoid stress on the fusion site until the fusion is healed. It also protects the neck from trauma after surgery. Too much motion at fusion site can lead to the fusion not healing. If one has good internal fixation of the fusion site, then the collar is optional.

It depends what on-: -physician choice & can also be driven by the # of levels fused as well as how securely the grafts are held in place. Also quality of bone of the patient. Stronger bones give better purchase of the bones & a stronger construct hence much more stable.
