Hard to tell: Swelling behind the ear with muffled hearing can be a sign of mastoiditis (an sign of a spreading severe ear infection). This is usually associated with pain. Cysts, lymph nodes and other masses can exist behind the ear. Either way, it is best for a physician to examine the swelling and to evaluate the ear as soon as possible.
