Answer: If the pain is in the back of the testicle without history of injury, most likely cause is epididymitis, which can often be diagnosed in a doctor's office by a simple urine test. Another common cause in young men for this type of discomfort is a varicocele, which is non infectious. Avoid heavy lifting, treat constipation/coughing as these things could increase the pressure and worsen the pain
