Immunodeficiency: I'm concerned enough about your recurrent infections that I would do an immunological work up. An Infectious Diseases doctor can help in this situation. The question is 'why' do you keep getting infected at a relatively young age? You need answers, and subsequently better treatment. Your ribs are likely hurting from continued coughing - costochondritis. Hopefully you can get help ASAP.
