Screw: If an ortho screw has moved, it can sometimes cause swelling and this can be felt. An XR will show if there is a change in position. All the best.

Loose screw: Hard to tell. Need an xray and examine the area. If this is where you hurt and the xray looks suspicious, then it's possible. GI see the doc who did your surgery.
