I have anterior shoulder pain with grinding sensation during internal & external rotation - occasional pain radiating down ulna. what could this be?

Many possibilities: was there trauma? is there pain associated with the grinding? generally grinding implies mechanical rubbing of non smooth surfaces. these can be articular such as cartilage or labrum for example. should be evaluated with MRI.
