Home Health: Wow, that is a tough question. Hospitals have an obligation to make sure ALL DISCHARGES ARE SAFE. The patient may need to be discharged for a brief stay in "subacute rehab" then home or with home services depending on insurance. The hospital will have a case manager to assist on discharge planning, which should have been started on admission. Ask to speak with the case manager ASAP!
