No drug interactions: There are no known drug interactions between dilaudid and clarithromycin - so yes, it is safe to take your dilaudid as prescribed. The half-life of clarithromycin is relatively short anyway - 5 to 7 hours - which means it's pretty much out of your system after 1 day. Hope you feel better.
