Many things: A delayed menstrual bleed may be cause ovarian cysts, thyroid problems, PCOS, stress, illness, being overweight, anorexia, medications, pregnancy, excessive exercise, breast feeding, fatigue, and inconsistent use of birth control pills. The cramping is usually due to your uterus trying to prevent heavy bleeding. The bottom line is this; if you are worried, see a doctor otherwise, observe.
