In real life, no.: The natural flow is downstream, which means a piece of condom (a slippery material) should flow out the vagina. The word "tiny" has no standardized meaning, so a piece that is tiny to one person can be big to another. Since people normally do not check condoms before and after sex, how would anyone know what size a hole in a condom was, and whether or not a piece actually came off during sex?
