May be GYN problem: Have your GYN MD check and make sure that you do not have uterine fibroids or other GYN pathology as they(sub mucous fibroids) can cause heavy bleeding in spite of pill use if they are under the lining of the uterus(endometrium). A ultrasound or hysteroscopy/laparoscopy can help in the determination.
