Inflammation, infect: The area around the roots is inflammed by bacteria, fungus etc. Psoriasis, seborrhea, follicultis or other conditions can cause swelling in the scalp and tenderness. Since this may need antibiotics or other treatment see md if medicated shampoos don't help.
