UTI? Herpes? Other?: Maybe the "sand" is pus, bacteria, etc. If your symptoms are typical for past UTIs, probably you have one again. But if at risk for STD, "excrutiating" pain on urination could be new herpes causing a sore or ulcer at the urethral opening. Assuming UTI, recurrences often can be prevented, e.g. with low dose antibiotic, especially after sex. Consider seeing an infectious disease specialist.
