Get evaluated: It sounds like possible a condition called a varicocele, which is a swelling of the veins in the scrotum. There are however more serious issues that need to be ruled out, such as testicular torsion. This depends on if this is a chronic condition or if happening for the first time today. If happening just now, go get immediate medical attention.
