Fissure?: It could be an anal fissure, which tends to be very painful with bowel movements. May also be a skin problem due to chronic diarrhea. Sounds like you need a proper evaluation and good quality exam to figure it out. See your primary care, and ask whether or not it's appropriate to get a referral to a GI specialist. Hope this helps!
