See below: Usually not, but generally speaking, enemas are soap with a sodium base. Water alone, theoretically, can cause electrolyte imbalances, but for one enema - likely fine.

WATER ENEMA: They used it a lot in the old days. I have not seen them use it lately except in hospital settings.

Potentially - yes: Water alone injected as an enema can be absorbed by the baby and dilute the balance of the blood.If enough water is absorbed, the baby can have seizures when this balance crosses a critical level. Small amounts (ounce or two) are unlikely to cause a problem, but i never recommend them.
