Hi i have pain in my left shoulder at the back and can feel pain near blades as well. i had high bp unstable . could it be heart related . i also take beta blockers what could it be.

Concerning: Depending on the degree of high blood pressure and the nature of the pain; this COULD be related to a new defect in the wall of the aorta due to shear stress from high BP. This is not common in comparison to other possibilities, but it would be an absolute emergency. Would at least call your doctor to report these symptoms and if s/he is unavailable, then head to the ER.
