A thought: More can be determined from physical exam and imaging, but I might start by ruling out degenerative spine disease, spinal stenosis or spondylolysthesis (or some combination thereof). The butt cheek/outer thigh involvement is suspicious for nerve entrapment (which would be worsened with upright exercise).
