Excellent Question: There are at least THREE Clinic Sites...The Original is in Rochester Minn. It is the ORIGINAL Site and I suggest that its the place to go if you can. Simple go to the Mayo Clinic Web Site or CALL the clinic and ask for INTAKE information. The Evaluation is AMAZING ...everything in ONE PLACE and OFTEN in ONE to TWO Days.
