Need exam. : Painless small volume bright red rectal bleeding is usually hemorrhoid in cause. Painful (rectally only) small volume bright rectal bleeding is often due an anal fissure. Abdominal pain &/or cramps with urgency &/or straining with larger volumes of rectal bleeding or bloody stools suggests proctitis &/or colitis. Weight loss, fever, age > 40, family history of colitis or colon cancer are worrisome
