What causes urine to be really foggy full of what i would assume is white blood cells and have a yellow greenish tint resembling limonade?

Infection: this could be infection. You need to have your physician Evaluate your urine and see if it's infected or see if there's another problem with the kidney that can be evaluated from the urine. After that you will need a further evaluation.

Just as you say..: Cloudy or foggy urine is due to particulate matter in the urine. This can be cells - white / red / cells from the lining of the bladder / or from crystals / or from debris generated in the tubules of the kidney. Very rarely stuff from the bowel or from the lymph system can show up in the urine as well.

Also if you do not drink enough water, Urine will be dark in color too.
