Two reasons: The most important reason is that mental disorders are poorly understood by comparison to other condition--however much we have advanced in our understanding. And what "we" do understand is not common knowledge. The other reason is that mental disorders affect how we relate to others so that it is easy for others to see not illness but something deliberate. We have a long way to go!
