Plaque vs. Occlusion: Large artery disease is plaque within the walls (not the openings) of the vessel. Small vessel disease typically refers to capillaries, ~5 micron id, closing off, due to plaque ruptures/clots shedding from the larger upstream arteries or capillary endothelial cell damage & closure most commonly seen in advanced diabetes mellitus. Drivers for both are similar: lipoproteins, blood sugar, bp, smoking.
