Here are some ...: If you have had recurrent pain as described with a high variable onset, duration, interval, evolution, progress, & resolution with associated groin upward pain and nausea, the first thing coming to mind is possible intermittent torsion (twisting) with spontaneous detorsion (untwisting) of the testis as well as possible kidney stone irritation in the upper of ureter/ renal pelvis.

Sounds like muscle: The pain you are describing sounds muscular in nature and there are muscle fibers called cremasteric muscles that originate from abdomal muscles around the spermatic cord and if they are sore from any source you will feel the pain in your testicles but not originating from the testicles
