Many reasons: epididymitis is a common cause of testicular pain in adults. It can be caused by inflammation of the epididymis. One easy way to help in that diagnosis is that the pain stops or gets better when you elevate the testicle. This can be caused by bacteria, STD's, virus. Testicular cancer can cause pain, urinary tract infection, kidney stones, inguinal hernia. It is best to see your doctor.
