Endometriosis?: Sometimes, back pain related to your cycle is due to endometriosis, or implantation of tissue from the lining of the uterus on organs in the abdomen, such as bowel, ovaries, bladder, etc. This tissue is hormonally stimulated and can cause cramping and pain with or after menses. See your doctor and get checked.

Not uncommon: It is not uncommon to develop posterior (back) uterine cramps right before, during, or right after your period. Try taking ibuprofen or naprosyn (naproxen). See medical attention asap if your cramps persist or intensity worsens.
