Kidney Pain: Pyelonephritis (upper urinary tract infection) is often accompanied with flank/back pain and fever in addition to urinary frequency and pain on urination. In the absence of fevers, chills, and urinary symptoms, urinary tract infection is unlikely to be the cause of your back/flank pain. Musculoskeletal causes are more likely. Recommend seeing your PCP if continues to have symptoms for >3-4 days
