Usually not: It's somewhat rare (although not unheard of) to use 2 extended release opioids at the same time unless you're topping out on the maximum dose of one or both of them. General rule of thumb is - in cancer related pain - 1 extended release and 1 short acting drug for breakthrough pain. Hope that helps!

Not good idea usually: Choose either one but not both unless you require a rescue drug. If that is case, might select a Fentanyl patch as the best approach.
