Small canal: Some babies are too big to come through pelvis which leads to a c section. Some babies fit through the pelvis but the canal does not dilate quickly or completely and the tissue stretch and may tear. The staff try to watch that progress and make a controlled cut-episiotomy to attempt to limit and allow good repair. Scar can form and tear at subsequent deliveries also. Generally the area heals well.

Several factors: While size of the newborn is a factor in whether or not a mommie gets a perineal tear other factors may contribute to tearing such as the baby's head in proportion to the pelvic opening--not just the vagina. Also, the ob/gyn or midwife's technique, the progression of labor, and the duration and intensity of laboring play a role.
