3 different things. : A fissure is a crack in the mucosa lining of the anus, very tender and painful with bowel movements. A fistula is a small tract or channel from the interior of the rectum to the skin around the anus or buttocks. An hemorrhoid is a bulbous vascular cushion on either side of the anal sphincter. They are managed differently. See a board certified general or colorectal surgeon in your are. Good luck.

Different problems: An anal fissure is a tear in the lining of the anal canal causing pain and bleeding. An anal fistula is a tunnel from inside the anal canal out to the perianal skin. It may cause drainage, pain and swelling. There are internal hemorrhoids which may prolapse and cause bleeding, discomfort and itching, and external hemorrhoids which may cause pain or difficulty with cleaning.
