Start with fiber: Fiber supplements will bulk the stool which can stretch the spastic internal sphincter that is the cause of fissure pan. Sitz baths also sooth the muscle. The addition of Diltiazem or Nifedipine or Nitroglycerin cream may ALS help. If medical therapy fails the surgery has highest cure rate.

An infection: In the perianal region, usually derived from the anal glands inside the rectum. A fistula is an abnormal communication between two organs, in this case the rectum and the skin outside the anus. Both require evaluation and often surgery to permanently resolve and prevent recurrences or complications.
