You've answered ??: You answered your own question. A majority of women have asymmetric breast size. If you are wearing a bra when you have pain in the larger breast--makes sense. The least expensive solution is by buy one cup size up.

Sounds mechanical: That sounds like a mechanical problem that could be solved by choosing a different bra. I would get into my regular doctor just be sure. Some stores have specialists for fitting bras.

Too tight??: Could it be that your bra does not fit right and is too tight and putting pressure on that side? That would explain why it does not hit when you take it off.
